Grace Hospice is currently caring for the needs of patients and their families in Illinois, Indiana, Michigan, Ohio, Virginia, and Wisconsin. Our CHAP accredited hospice offers a family centered model of care, focused on quality of life for those facing a life limiting illness. Our interdisciplinary team consists of physicians, nurses, hospice aides, social workers, chaplains, bereavement coordinators, therapists, and volunteers. We provide our services in private homes, long-term care facilities and assisted living facilities.
