Legacy Medical Foundation is a 501(c)(3) focused on supporing low-income seniors and their caregivers across Southeast Michigan. We provide microgrants to support transitions from the acute care setting to other levels of care. Microgrant funds support a variety of needs, such as, private duty-home care, purchasing and installing grab bars and other durable medical equipment, nutrition, and non-emergency transportation. We are able to measure each grantee's change in health and wellness as a result of receiving supportive services.
