Our History Granite Care Homes, Inc. has provided residential services to individuals with unique needs since 1964. Granite Care Homes started as an Intermediate Care Facility and since has evolved to providing services to our clients in adult foster care. Granite Care Home, Inc.’s mission is to provide exceptional home and community-based services to the persons we support so they can achieve and maintain the lifestyle of their hopes and dreams. We strive to provide these services in a professional, courteous, and caring manner. Site Locations We have six homes in the Sauk Rapids area within 1 mile of each other.
