Whānau Tahi Connected Care brings to life the promise of connected care for communities and clinicians. Our software and services enable health and social care professionals from multiple organisations and disciplines to work as a team to deliver patient and whānau-centred care. Our organisation is an amalgamation of the pioneering whānau-centred care platform Whānau Tahi Navigator, and the purpose-built connected care platform Whānau Tahi Connected Care (formerly HSAGlobal’s CCMS). Combined, we enable and promote better, more cost-effective health and social outcomes for funders, patients and whānau.
