Working in collaboration with General Practitioners, Community Pharmacies and other healthcare team members, The Medicines Review Company provides quality Home Medicines Review (HMR) reports for individuals in the community and Residential Medication Management Review (RMMR) reports for residents in government funded aged care facilities. The Home Medicines Review (HMR) and Residential Medication Management Review (RMMR) programs are funded by the Australian Government and is designed to assist individuals to maximise the benefits of their medication and reduce or prevent medication-related problems. The medication review reports provided by the Medicines Review Company are written by experienced and qualified Consultant Pharmacists who are accredited with the Australian Association of Consultant Pharmacy (AACP).
