Be Healthy at Home, LLC. is locally owned and operated Home Health Agency in Buda, Texas. Our staff specializes in a higher acuity population, clients that tend to have multiple diagnosis' putting them at risk for re-admissions into the hospital, therefore requiring better coordination of care prior to them leaving a facility. With multi-level communication that takes place across all levels of care; in real time, we are able to keep clients out of the hospital by assessing their immediate needs. Our partnership with Post Acute Medical allows us to offer our clients the full continuum of care. Our navigation teams work closely with the providers in the community to ensure risk assessment occurs after an extended hospital stay or when a new diagnosis takes place. Prior to our first visit we are planning and educating our clients and their caregivers of their individualized care plan. Our staff guides and encourages the caregivers and clients to establish independence in their home, while respecting privacy and maintaining their integrity.
