Ubiqare Health Pvt Ltd is a technology-based healthcare start-up company, that pursues a vision of making healthcare ubiquitous, easily accessible, collaborative, and smart. Ubiqare has been founded by 5 accomplished senior professionals – 3 technologists from IITB and 2 healthcare leaders. We extend follow-up specialty supportive/palliative care from the hospital to the post-acute and chronic care patients at home. We combine clinical telepresence with clinical workflows and network of clinicians to deliver follow-up medical care to the patient at home. We enable specialists and hospitals to extend their follow-up care to their patients after discharge and achieve the desired treatment outcomes they want to see in their patients. To the patients, we are the all-pervasive healthcare enabler for follow-up care to receive continuity of care from his trusted specialists/ doctors from the comfort and convenience of his home. We have been serving patients since Jul 2018. We have served > 350 patients so far directly , with high severity care needs, suffering from cancer, stroke, MS and post-operative rehab. We have delivered around 12000 patient-days of care and have clinically managed >90% of episodes/patient/month at home. We are laying the foundation that virtual hospitals will be built upon.
