CHIOC helps Orange County residents access health and social service programs by providing outreach, education, enrollment, and case management services to vulnerable populations. We strive to ensure that clients acquire, retain, and utilize community resources effectively. CHIOC has been providing the Community Health Access Program throughout Orange County for more than a decade. Since inception in 2006, we have linked more than 42,700 children and adults with public or private health care programs and conducted more than 162,000 case management follow-up services to confirm medical and dental homes.
