Our mission is to empower individuals with disabilities to achieve goals and live their most independent lives. To accomplish our mission, Bridges engages in a process of discovering individual needs as well as individual dreams. We want to ensure informed choice and person centered planning in helping to design innovative service models for individuals no matter the level of need. Presently Bridges has CRS homes, GRH housing, provider owned apartments and landlord relationships for creative housing options. This continuum of care will give individuals a large sampling of housing options in regards to location, affordability, unit mix and accessibility. Bridges provides licensed services such as Corporate Foster Care, SLS, ILS, SES, 24 Hour Emergency Assistance among others. These services will allow for individuals and their teams to choose a service plan that fits the specific needs and wants in the community of their choosing on an a la carte basis. Bridges has a strong interest and track record in serving individuals transitioning from institutional or unsuccessful placements.
