Foundations Psychiatric Rehabilitation Program (PRP) and Medical Adult Day Care are designed to meet the needs of individuals (ages 18 and older) who have co-occurring disorders including chronic health conditions, mental illness and substance dependence. Foundations helps facilitate a person's recovery, including the their ability to make decisions while creating opportunities regarding home, school or work and community.
