We provide end of life care that encompasses the physical, social, psychological and spiritual needs of our patients and families. We resolve to be the hospice provider of choice for our patients, families, employees, investors and for our community. Legacy Hospice provides care through an Interdisciplinary team. This team consists of the Registered nurse, Physician, Social worker, Chaplain, Dietician and the Volunteer. Members of the Interdisciplinary team are considered "core" services by Medicare, and must be provided by employees of the hospice.
