Concierge at Home provides a single resource for clients and their families to simplify the overwhelming task of maintaining independence. A thorough evaluation of needs, creation of a care plan, assistance with implementation and ongoing management of the services are the keys to successful independence. Assessment – At the heart of the Concierge at Home program is the needs assessment. Our Care Managers, Physicians and Pharmacists meet with the client, family and caregivers to gather information and identify needs. These in-home assessments provide a comprehensive evaluation of every aspect of the client’s current situation and barriers to independent living. Planning – The individualized care plan encompasses all of the information gathered through the in-home assessment. Under the guidance of our geriatrician advisors, a personalized care plan is created and a package of services is selected from our preferred network of providers. 

Implementation – Our Care Managers assist in organizing and implementing the recommendations of the care plan. Concierge at Home coordinates the pieces of the solution outlined to maintain independence in the community – whether at home or in a facility. Management – The Concierge at Home team continues on-going management and coordination of the care plan to assure effectiveness and client satisfaction. As needs change, the plan is updated and services adjusted to meet those needs.
