Physical Medicine and Rehabilitation clinicians partnered with Information Technology specialists to create a tool that can share clinicians` expertise with patients. The Connected mHealth platform assists in exercise building and prescription while enabling digital tracking of the patient’s progress and feedback through the whole rehabilitation process. The platform software works with many wearable devices enabling the patients to independently track their exercise treatment plan.
