HCI transform pathways of care with digital information. By producing digital solutions that engage with patients and clinicians to improve patient education, we work with health care organisations to make significant reductions in outpatient appointments and are one of only a few digital health organisations that have been awarded a national NHS contract. Spanning multiple specialties including cardiology, maternity, physiotherapy, and podiatry, we have helped achieve an annual saving in one NHS Trust alone of £350,000. One such solution is our CONNECTPlus app that is the only co-morbidity tool that allows hospitals to support patients to manage their multiple conditions remotely and in one place. In addition to information provision, CONNECTPlus provides patients with the ability to record and track their symptoms, medications and appointments. This supports enhanced compliance, remote management and enables patient-initiated follow-up. HCI has also produced and implemented the largest collection of health and care information videos in the UK and plays a key part in developing new technological solutions for the NHS, including the national health and care video library made free for NHS use during COVID-19 by NHSX.
