This approach can lower the overall cost of health care for higher risk populations. This has already been convincingly demonstrated among higher risk chronically ill older adults. As tested within the Medicare Coordinated Care Demonstration and with Aetna, HQP’s model of Community-based Advanced Preventive Care, significantly reduced hospitalizations, deaths, and total Part A & B Medicare expenditures. HQP is now inviting organizations to consider becoming strategic partners and early adopters of our model. The general concept of preventive care services is not new, but a systematic program that reliably delivers a robust set of such services to higher-risk subgroups resulting in better population health and lower health care costs is a breakthrough innovation.
