The Durham Dales Health Federation is a formal collaboration of the 12 GP practices in the Durham Dales area. We provide enhanced services for our patients, and aim to maintain and develop the provision of healthcare as we approach an increasingly challenging future. Primary care federations are becoming an increasingly important part of the delivery of NHS services, and DDHF is at the vanguard of this new model. DDHF aim to develop new and innovative approaches to delivering healthcare, leveraging expertise from different sectors as well as emerging technologies to provide effective and efficient solutions.
