Because our healthcare system is so fragmented and the management of care is so complex and time consuming, we devote our time to doing this, expertly - being nurse practitioners - bringing everyone together and in the know about symptoms, data, and treatment plans. As clinicians, we are able to work with your care team to identify problems/potential problems, create medical plans, and then creatively work to execute/ implement these plans in the home by bringing in the resources needed, or bring the patient to the resource.
