Optim’Care has launched a challenging project of using digital technology to deal with the current transplantation issues (organ shortage, loss of graft function, limited resources, etc.). More precisely, our aim is to develop solutions to facilitate the implementation of precision medicine in transplantation in order to: - improve graft survival and the quality of life of transplant patients : - optimize the time spent on healthcare by professionals. Our platform currently integrates remote monitoring and e-learning solutions. Meanwhile, it is constantly expanding thanks to the creation of an ambitious R&D programme aimed at developing: - Solutions to support prognostic and diagnostic based on technological innovations (Artificial Intelligence). - Solutions to empower patients by means of organizational innovations for facilitating and putting them at the centre of their healthcare. Our solutions are designed with both healthcare professionals and patients in order to meet their needs in the best possible way and to permit optimal implementation and accessibility to all those involved.
