OHL is the integrated community provider working across SEL, delivering a diverse set of services for practices, PCNs and patients. OHL was established to help provide a range of primary and community care services to residents as well as several ‘at scale services’ for practices. Historically this was to : • ensure primary care was represented amongst commissioners and stakeholders • to strengthen clinical governance and improve the quality and safety of services • efficiencies of scale and scope, for example in back-office functions or the procurement of practice services • strengthen capacity to deliver services • to develop training and education capacity • improve local integration across practices and other providers, whilst supporting personalised care • shared responsibility for development and delivery of high-quality patient-focused services Primary & community care remain complex environments, subject to frequent changes within the commissioning landscape. We are proud of our dynamic & innovative team who work flexibly and responsively with System partners to deliver services ‘at scale’ (many directly to patients) within the Integrated Care System. Our priorities for 2022- 2027 are to: Support practices & PCNs, connect across London, deliver community- based services, ensure sustainability.
