Brilliant Health Outcomes has developed My Patient Monitor (MPM) which is a remote patient monitoring platform that connects patients to providers and care teams through scheduled questionnaires and educational content curated by the provider/care team. These questionnaires are standard or customizable to the needs of the patient. Using connected and remote patient monitoring has shown to increase the level of care and collaboration between patient and provider and peace of mind for the patients that are safe at home or place of choice.
