Helping Hands Senior Foundation offers a care planning / care coordination to seniors regardless of their income, insurance or health condition. Each month we assist several hundred seniors with learning about patients rights in a nursing home, tenant rights in the communnity, setting up affordable care at home, health services, VA programs, fall prevention, mobility products and other various assistance programs to help promote independence.
