We Provide Care Management for children suffering from emotional trauma, chronic medical condition, mental disorder, anxiety, loss of a loved one, abuse/neglect, parental separation or divorce. Our Care Managers will help by coordinating care and services in order to improve healthcare outcomes as well as quality of life by leading them to the support they need and help register for the programs they are eligible for.
