Proactive Health Advocates provides Registered Nurse healthcare navigation and advocacy services to individuals and families and employees/members of businesses and unions. We are hands on, in-person, ensuring that the patient understands and has input into their care and that the care is coordinated and necessary. We accompany patients to their appointments, collaborate with providers and help coordinate care if hospitalized, and work with the entire healthcare team to ensure team communication. We are experts at navigating and coordinating individual and employee benefits to maximize appropriate use while decreasing unnecessary care and costs. We take the confusion out of our healthcare system.
