LIFE ElderCare offers a unique, innovative model for community-based aging in place. Using a cadre of fully screened and trained volunteers, LIFE ElderCare provides low-to-no-cost personalized services that help older adults stay connected to health, resources & community. Our direct service programs offer a community-supported option for healthy, independent aging. Meals On Wheels delivers nutritious mid-day meals, Friendly Visitors provides caring companionship, Fall Prevention makes home environments safer for seniors, and Transportation provides rides and destination assistance for medical appointments and other essential errands. For those with more complex needs, we also offer Care Coordination throughout Alameda County. Our programs create both a safety net for people with chronic or debilitating illness, as well as a springboard for the adoption of healthy lifestyle habits that will enhance longevity. And, we provide these services outside of the traditional fee-for-service model. Although our vision is simple, it runs on the power of thousands. This agency owes its effectiveness to the combined efforts of faith-based centers, legislators, health care providers, schoolchildren, colleges, city workers, local businesses and banks, membership clubs, sister non-profits, individuals, the families of seniors, and of course, the seniors themselves, who enrich our lives with their amazing stories, gratitude, and love.
