We do this by calling them throughout the day and, if they answer, providing a personalised reminder or check-in. If they don't answer we can inform those supporting them that they should check-in. We have helped over 2000 people and delivered over 900,000 reminders/check-ins to date. We have also saved over £1.5 Million for the public sector through improved social worker allocation, reduced admission to hospital and reducing bed blocking.
