Take Control is a health care company that works with self-funded health plan sponsors, data aggregation companies, and healthcare consultants & service providers. Typically, incentives are offered in the form of increased or reduced employee share of medical premiums, reduced or waived prescription costs, reimbursed gym memberships, and other methods. We contact high risk plan members to let them know about the incentives, and the benefits of improving their health. All plan members must meet established criteria to qualify for enrollment in our program. Our unique telephonic delivery method allows clients to participate from work or home, and receive the individual attention needed to ensure success. Our health coaching includes intensive one-on-one monthly phone sessions with licensed dietitians, exercise and sports science trainers, and certified diabetes educators. Our Health Coaches are trained in nutrition, health education, corporate wellness, and behavioral health. Your plan members will be coached by our professionals based on your plan member's needs, as determined by their custom coaching plan.
