Welsh Mountain was founded in 1969, and currently serves Lancaster, Lebanon, Berks, and Chester from five different locations. We are registered as a Patient-Centered Medical Home. As a Community Health Center, Welsh Mountain Health Centers is a safety net provider of integrated medical, dental, and behavioral care, as well as outreach assistance for health care coverage and connection to social services to anyone in the community, especially those with barriers to care. 93% of our patients are below 200% of federal poverty guidelines and 54% live in poverty. 70% of our patients are covered by Medicaid or CHIP.
