Primary Health Services Center (PHSC) is a private, non-profit (501(c)(3) clinic founded in Monroe, LA in 1997 by local citizens. As a Federally Qualified Health Center (FQHC) for over 20 years, PHSC is an organization involved in community based holistic healthcare and has expanded its provision of services to include Ouachita, Morehouse and Lincoln Parishes. PHSC is recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH). In addition to the high quality, culturally competent healthcare provided, PHSC offers supportive services such as health education, case management, translation, and transportation services to increase access to healthcare.
