Community Care Concepts of Woolwich Wellesley and Wilmot is a non-profit, charitable, volunteer-based organization that supports seniors and adults with unique needs to live independently in their own home. We provide a continuum of services and supports that respond to the needs of seniors and others with unique needs living independently across Woolwich, Wellesley and Wilmot Townships. In addition to receiving referrals from other service providers, individuals or their family members can call us directly at 519-664-1900 or 1-855-664-1900 to speak with one of our staff. We would be happy to help arrange services or refer to other community partners who may better address needs identified. Our staff are also willing to visit individuals in their home to understand an individual's needs and to determine how we can best help .
