The Care Manager helps manage the complex issues associated with aging that maximizes the senior’s independence and safety, and honors their individual wishes, by creating a plan of care that specifically meets the older adult's needs. Having the right Care Manager can help contain costs as well as improve quality of life for older adults as well as families. The Care Manager can come in, assess the client's needs and make recommendations. The Care Manager can also provide continuing support for situations that cannot be resolved quickly, or require ongoing attention.
