Elevate Health Group officially opened its doors in August of 2015. Our brand new state of the art office is located in Downtown Glendale. Our aim is to provide our patients with the highest level of care, with an evidence-based and modern approach. Our group is currently comprised of six physicians and a nurse practitioner. Elevate Health Group is founded on the principles of a traditional Internal Medicine practice. Our patients' care will be continuous by their regular physician, whether in the outpatient setting, hospital setting, or in need of a skilled nursing facility. Our physicians value continuity of care and hold the belief of it to be the basis of effective medical practice.
