Subacute Rehabilitation - Our therapy department is directed by a PHD in Physical Therapy who works with you to develop a comprehensive plan focused around getting you home as soon as possible. Our Activities of Daily Living Suite features modules for training in kitchen, bedroom, bathroom and laundry settings to help you regain the skills you’ll need to be successful upon returning home. Long-term care - Choosing to admit a loved one to a long-term care facility is an emotional and life-changing decision. At Autumn Woods, we understand the difficult choices you’re facing, and through guidance and support we’ll help you take the next step. Ventilator Dependent Unit - Autumn Woods offers the only dedicated Vent-Trach unit in Macomb County with a focus on providing specialized services for weaning and independence. Respiratory services - In addition to our ventilator dependent unit, we provide respiratory services to tracheostomy, COPD and CHF patients who require close monitoring. Our respiratory staff is in the building 24/7 and can assist anyone with breathing difficulties. Preventing rehospitalization is one of our highest priorities. Dialysis services - Our on-site soft dialysis program provides patients with more freedom and improved clinical outcomes. Memory care - Our dedicated memory care unit for individuals with Alzheimer’s and dementia is designed to allow residents to ambulate freely in a controlled and secure environment. Our programs are tailored to meet their needs while keeping them active and engaged.
