www.ccfcwi.orgCCFC’s guiding principle is that, together, we can eliminate the obstacles to health care in our community, one patient at a time. There is no charge to see our providers, and prescription medications are provided at little or no cost as needed. Our work is accomplished by 127 medical and lay volunteers and two part time staff people, and is supported generously by donations from local individuals, churches, businesses and organizations, as well as grants and fundraising events. CCFC is in downtown Dodgeville, across the street from the Public Library and City Hall. The entry is handicapped accessible. Through the double glass doors you will see a reception area and comfortable waiting room. We have four exam rooms, a lab, pharmacy, prescription assistance consult room, social care room, and a kitchen and conference area for specialty services such as mental health counseling and yoga therapy. CCFC is supported largely by its surrounding community. About 1,000 individuals and families have given one or multiple donations. Iowa County churches regularly submit special collection or special event donations. Many local businesses and service organizations support our work. Grantors during our four years of existence have included Alliant Energy, Salvation Army, United Fund, Iowa County Board, Morton Foundation, Mineral Point Community Chest, Biddick Foundation, Dean/St. Mary’s Health System, and Wisconsin Medical Society. Since opening in October 2006, CCFC has seen more than 2,800 individual patients. Those patients have made 14,646 visits to the clinic. We see 10-12 patients each evening the clinic is open. Current Prescription Assistance Program enrollment is about 40 patients. Average annual prescription savings per patient is an estimated $4,400. Estimated total annual prescription savings for our patients is about $176,000. We have 127+ active volunteers, who have given 4,000 hours of direct service in the past 12 months.
