Person-assisted telehealth hosting. We are a tech bridge to care. We are non-medical providers who create access points for chronic condition, post-operative, post acute/discharge management for community members. We have a brick and mortar footprint to provide hosting for communities with underserved populations or areas, especially as it relates to specialty providers. We close gaps in access to existing networks and provide opportunities for new networks. We address limitations associated with broadband insufficiency, health, general, and technology literacy challenges, and lower income populations who struggle to travel to distant providers. We are staffed with Community Health Workers who bridge the gap for community members. We leverage certified EHR technology that is ONC certified, collect vital signs data, and individual level SDOH data.
