Palliative RX specialized palliative care and pain management care delivery model was the first in NJ to provide care in skilled nursing facilities and has reduced facility hospital readmission rates, improved pain scores, and increased overall patient satisfaction scores. We provide webinars to patients and families to give them control of their serious illness, allow them to effectively manage symptoms, and prepare for what comes next.
