HealthQuilt is an interdisciplinary practice association comprised of primary care physicians, nurse practitioners, pharmacists, dieticians, specialists, allied health professionals, and health educators. HealthQuilt utilizes the patient-centered medical home (PCMH) model of care which assures care is coordinated, accessible, safe, and effective. Through a robust system of health information management, HealthQuilt is able to keep all providers on the same page. HealthQuilt also offers an employer onsite program where medical services are brought straight to your workplace.
