Longevity Health Plan addresses a significant unmet need of the institutionalized Medicare Advantage members who reside in nursing homes through a more person-centric model of care that improves coordination of care and reduces avoidable hospitalizations for members placed in the institutional setting. Core to our approach is the onsite full-time placement of high performing primary care practitioners whose mission is to ensure comprehensive care plans that take into account each member’s medical, psychosocial, cultural preferences and personal goals.
