Careways was created to extend care beyond the physician office. In the Careways Program, we seek to revive the heart of healthcare by supporting member’s everyday health needs as well as the relationship with Primary Care Physicians. We believe that relationships are at the heart of excellent healthcare. Our team of care coordinators, nurses and pharmacists are here to support members in maintaining a healthy lifestyle, walking through difficult health events, managing chronic illness or providing decision support for everyday health needs. In addition, we support member’s engagement with their Primary Care Physician. In addition, our team is specifically equipped to support members with the following health needs or conditions: Diabetes, Asthma, COPD, Obesity, Heart Disease, Hypertension, Kidney Disease and Cerebrovascular Disease Also members transitioning from a hospital or care facility to a home setting and members facing major health events We’re excited to help members thrive!
