Imago Community is a social action charity who support upwards of 30,000 of the most vulnerable individuals in Kent, East Sussex, Medway and London. A hugely varying charity in the work we do, Imago support both young and older people in a variety of ways. Support for Children & Young People includes: • Young Carers activities across Kent, East Sussex, Bexley, Havering and Southwark, • Short Breaks activity programme supporting disabled children and their families across Kent, • YANA (You Are Not Alone), a mental health programme for children &young people across Kent, • Safe, a programme that supports the mental health and wellbeing of young people by working with them in their schools and communities in Kent, • Young Adult Carers support across Dartford, Gravesham and Swanley, • Social Prescribing for children . Support for Adults includes: • Wellbeing Matters Plus; support for adults aged 55+ in Ashford, Canterbury and Swale, • Wellbeing Matters Care Navigation Kent, supporting individuals in Dartford, Gravesham, Swanley and Swale, • Medway Wellbeing Navigation Service, supporting individuals aged 18+, • Adult Carers support in Dartford, Gravesham, Swanley and Swale, • Dial2Drive community transport scheme in Tunbridge Wells and surrounding areas, • Social Prescribing for adults throughout Kent.
