Blaze Advisors was formed in 2016 to offer a suite of talent and tools to help healthcare partners optimize their pay for performance programs. We take a pragmatic innovation approach to identify realistic and impactful solutions for our customers. Services often included: - assembling and analyzing disparate population health data, - implementing and automating care management programs, - and designing and deploying communal disease/risk screening tools. In 2017, Blaze Advisors was asked to build and manage integrated care networks as a core project, now known as ONEcare, that coordinate the historically siloed medical and behavioral health communities. Using straightforward communication protocols and tools, ONEcare networks seek early identification of the disease, timely access to care, and strong treatment activation and patient engagement. Today, Blaze Advisors is a multi-functional advisory and management team working with hospitals, emergency rooms, primary care, behavioral health, and post-acute care partners across the world.
