About Us: Visiting Counselors of New York, LCSW, PLLC (VCNY) specializes in providing in-home services to seniors and disabled adults. We offer in-home counseling, care management services, and long-term planning services throughout Queens and Long Island. It is VCNY's hope that by bringing services to the individual and connecting people with dedicated and caring social workers, more people will receive support and assistance. Overview of Services: In-Home Counseling: Our counseling services are offered in 30, 45 and 60 minute sessions. All of our practitioners are licensed mental health professionals and we accept Medicare. Care Management Services: Review entitlement eligibility and assist clients in obtaining all appropriate entitlements Assist in the completion of Medicaid and SNAP recertification. Provide assistance in determining options for long-term plans. Assist in locating and applying for senior housing. Act as a liaison and advocate between our clients and: courts, family, doctors, insurance companies, Social Security, hospitals, and other systems that support and affect the welfare of our client. Maintain contact with any identified significant individuals. Act as advocate for our clients wherever necessary. Provide family and caregiver support. Long-Term Planning: Our long-term planning services offers clients a comprehensive array of services to help address their current and future needs. Some of these services include assistance in applying for various entitlements and programs, including Medicaid, assistance applying to senior housing, completion of Health Care Proxy, Living Wills, and assistance in addressing out of pocket prescription cost. VCNY also helps our client's with considering all options when helping them to stay in their home, including obtaining home health aide assistance.
